Dynamic Electromyographic Activities of Scapulothoracic Muscles in Patients with Frozen Shoulder Syndrome
Abstract
Objective: To investigate the pattern of scapulothoracic muscles activities during arm elevation in patients with frozen shoulder syndrome using dynamic electromyography (dEMG). Methods: dEMG activities of six scapulothoracic muscles; upper trapezius, middle trapezius, lower trapezius, serratus anterior, anterior deltoid, mid-deltoid in 20 patients with unilatreal frozen shoulder syndrome were measured at affected and unaffected shoulders. Data were gathered at six different shoulder positions: 60..and 120..flexion, abduction in coronal plane, and abduction in scapular plane. Results: The affected shoulder revealed increased upper trapezius activity at all positions compared to the unaffected shoulders and lower trapezius activity at all positions except 60..flexion (p<0.05). Serratus anterior activity at all positions except 60..abduction (p<0.05) and mid-deltoid EMG activities were increased at 120..flexion and abduction in the scapular plane (p<0.05). However, in the middle trapezius, anterior deltoid EMG activities there was no significant difference between the affected and unaffected shoulder. The RVC (reference voluntary contraction) ratio of upper trapezius/lower trapezius and upper trapezius/serratus anterior at all positions were more increased in affected shoulder. The affected shoulder appeared to have greater increases in upper trapezius, lower trapezius, serratus anterior EMG activities in the 120..positions than 60..positions. Conclusion: The results showed that the limited glenohumeral joint motion in frozen shoulder induced overactivity on the scapulothoracic muscles related to the scapular elevation and lateral rotation, and more at the higher arm elevation.
